Obesity, type 2 diabetes and non-alcoholic fatty liver disease (NAFLD) are closely associated, but the causal relationships between these metabolic diseases are not always clear. Using genetic data from 2 large cohorts, as well as transgenic mice with the well-known PNPLA3-I148M variant (as a typical model), Liu and coworkers demonstrate that NAFLD can cause diabetes, while diabetes and obesity can cause NAFLD. These data may provide the basis for a better subphenotyping of patients, although the relevance for improved diagnostics and tailored therapeutic approaches remains to be determined.
